Vasona Creek Healthcare is seeking a dedicated and passionate Occupational Therapist (OTR) to join our short-term rehabilitation team.

Our therapists play an important role in helping patients regain strength, functional skills, confidence, and independence following events such as joint replacement, cardiac events, strokes, and other acute medical conditions.

Your Role as an Occupational Therapist

As an OTR, you'll guide patients through their rehabilitation journey and help them achieve their highest possible level of function and independence.

Key responsibilities include:
  • Evaluate patients and develop individualized treatment plans in accordance with physician orders
  • Provide skilled occupational therapy services and supervise Occupational Therapy Assistants in accordance with the California Occupational Therapy Practice Act
  • Document evaluations, progress notes, treatment sessions, and other required information accurately and timely
  • Collaborate with nursing, therapy, physicians, supervisors, and other interdisciplinary team members
  • Participate in patient care conferences, utilization review meetings, and rehabilitation conferences
  • Educate nursing staff, patients, and families on restorative programs, caregiver techniques, and strategies to support continued progress
  • Recommend and assist with obtaining appropriate durable medical equipment (DME) to promote safety and independence
  • Participate in discharge planning to support a safe and successful transition to the next level of care
  • Assist with facility in-service training and staff education programs
  • Monitor therapy equipment and promptly report maintenance or safety concerns
