About the role:

Southlake is recruiting for an Occupational Therapist or Physiotherapist to support the Aging Well Clinic. The Occupational Therapist or Physiotherapist is an integral member of the clinical care team, providing direct support to the clinic's Geriatricians and Nurse Practitioners. The Occupational Therapist or Physiotherapist will provide comprehensive assessment, consultation and counselling for complex seniors in an interprofessional outpatient clinic setting while developing and maintaining partnerships with our community providers.
What you'll bring:

• Registered in good standing with the respective College/Regulatory Body (College of Occupational Therapists of Ontario or College of Physiotherapists of Ontario)
• Masters of Occupational Therapy or Physical Therapy or equivalent from a recognized university
• Minimum 2 years of experience working with the geriatric population preferred
• MOCA certification required, or willing to obtain upon hiring
• BCLS required, or willing to obtain
• Demonstrates excellence in communication, both written and verbal
• Demonstrates ability to work as a collaborative, positive and inclusive team member
• Demonstrates a commitment to delivering patient and family inspired care
• Possesses advanced assessment and management skills, including both physical and cognitive domains
• Possesses working knowledge of geriatric syndromes and community resources applicable to the geriatric population
• Demonstrates initiative, willingness to learn, and dedication to lifelong learning
• Demonstrates a commitment to clinical innovation and evidence based practice
• Demonstrates working knowledge of the Regulated Health Professions Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act
• Demonstrates commitment to, and understanding of the principles of diversity, equity and inclusion
• Demonstrates superior problem-solving abilities and critical thinking skills
• Proven ability to attend work regularly as per the Attendance Support
