Responsibilities

  • Conduct initial assessments and reassessments of participants' conditions

Occupational Therapist

Department: Therapy

Employment Type: Full Time

Location: St. Joseph, MI

Compensation: $80,000 - $90,000 / year

Description

Schedule: 40 hours per week | Monday - Friday | 8am - 4:30pm
Department: PACE of Southwest Michigan | Niles, MI & St. Joesph, MI

What You'll Do:Join a mission-driven team where you can make a meaningful impact while enjoying a strong work-life balance. The Occupational Therapist provides participant-centered evaluations, treatments, and interventions that help participants maintain independence, safety, and quality of life at home, at PACE of Southwest Michigan, and in the community. As part of the interdisciplinary care team, this role contributes to individualized care planning and ongoing coordination to support successful community living and aging with dignity.

Key Responsibilities
  • Conducts and documents initial assessments and reassessments of participant's condition and functional status.
  • Develops treatment programs when indicated, sets goals and provides treatment.
  • Provides occupational therapy as determined by the assessment.
  • Reviews assessments and discharges participants when appropriate.
  • Assesses the need for adaptive and durable medical equipment.
  • Communicates participant changes to Interdisciplinary Team (IDT) members.
  • Coordinates with the Home Care Coordinator to purchase durable medical equipment.
  • Participates in care conference.
  • Completes home visits when directed.
  • Provides training and education to family members, friends, and PACE of Southwest Michigan staff members for those parts of the treatment plan that can be performed by them.

Skills, Knowledge & Expertise
  • Active and unencumbered Occupational Therapy license in the state of Michigan.
  • Two (2) years' experience as an OT, required.
  • Valid Driver's license and reliable transportation.
  • CPR certification or willing to obtain.
