OBAT Nurse Care Manager
Pay: $98,000 to $113,000 per year


Location: Roxbury, MA

Job Summary:

The OBAT Nurse Care Manager plays a key role in the care and management of patients with substance use disorders (SUD) receiving office-based addiction treatment (OBAT). This registered nurse serves as the central point of contact for patients enrolled in medication-assisted treatment (MAT) and coordinates care among providers, behavioral health clinicians, and community resources. The Nurse Care Manager ensures evidence-based, patient-centered care that promotes recovery and overall wellness.

Key Responsibilities:

Clinical Assessment and Patient Care
  • Conduct comprehensive nursing assessments of patients initiating or continuing MAT (e.g., buprenorphine, naltrexone).
  • Monitor patients• clinical response, side effects, and adherence to treatment through regular follow-ups.
  • Provide patient education on MAT, withdrawal symptoms, harm reduction, and relapse prevention.
  • Administer or oversee administration of medications as needed; coordinate with prescribers on treatment adjustments.
  • Perform and interpret toxicology screening and coordinate care based on results.

Care Coordination and Case Management
  • Serve as the primary care manager for OBAT patients, ensuring continuity of care across settings.
  • Develop and maintain individualized care plans in collaboration with the care team.
  • Coordinate referrals to behavioral health, primary care, social services, housing, and recovery support services.
  • Assist with insurance authorizations, prior authorizations, and medication access barriers.
  • Care coordination with other Dimock Addiction levels of care including ATS, CSS and Askia.

Team Collaboration and Communication
  • Work closely with physicians, nurse practitioners, recovery coaches, and counselors to ensure holistic care.
  • Participate in interdisciplinary team meetings, case reviews, and performance improvement initiatives.
  • Maintain accurate, timely documentation in the electronic health record (EHR) in compliance with regulations.

Program Development and Quality Assurance
  • Support program metrics, data collection, and reporting for regulatory or grant requirements.
  • Contribute to development and implementation of policies, procedures, and clinical protocols related to OBAT services.
  • Educate clinic staff and patients on addiction care practices and emerging trends in MAT. Assists in other projects such as Addiction related clinic grants.
  • Collect data for the bureau of substance addiction services on new patients, quarterly and at discharge: submit this data as required


Qualifications:
  • Licensure: Current and unrestricted Registered Nurse (RN) license in Massachusetts
  • Education: Associate or Bachelor of Science in Nursing (BSN preferred).
  • Experience:
    • Minimum 2 years of nursing experience; experience in addiction treatment, behavioral health, or community health strongly preferred.
    • Familiarity with MAT protocols (e.g., buprenorphine, naltrexone) and trauma-informed care.
  • Certifications:
    • BLS required.
    • CARN (Certified Addiction Registered Nurse) or similar credentials preferred but not required.

Salary: The approved range for this role is 84,000.00 • $113,000.00
