JOB SUMMARY:
Works under the supervision of the Branch Nursing Leader. The Nursing Team Leader will be responsible for coordinating initial nursing care for infusion patients and monitoring the ongoing home health nurse/patient relationship and patient progress. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. The Nursing Team Leader will work closely with the intake department, clinical pharmacists, sales team, administrative departments and outside agencies.
MAJOR DUTIES AND RESPONSIBILITIES:
1. Coordinates with intake department to obtain appropriate clinical information and coordinate nursing care for new infusion patients.
2. Monitor scheduled nursing visits for infusion patients.
3. Serve as primary contact/liaison between KabaFusion branch and various home health nursing agencies.
4. Develop cooperative relationships with home health agencies, referral sources, KabaFusion billing department.
5. Participates in providing orientation and clinical competency for new nursing staff and field competency visits as needed.
6. Works cooperatively with all professional health members, conducts team conference and provides for continuity of care and coordination among disciplines and other departments.
7. Assures accurate, current medical records are maintained according to state and federal requirements, maintains patient confidentiality.
8. Acts as liaison between the physician, nurse, pharmacist and patient/family when problems arise.
9. Works cooperatively with other home health agencies and/or infusion agencies in providing authorizations for skilled nursing visits.
10. Assists with patient scheduling needs.
11. Assist with "on-call" needs of the agency
12. Evaluate and intervene in nursing-related patient care issues.
13. Other duties as assigned
SKILLS AND ABILITIES:
• Ability to organize and prioritize work with attention to detail
• Good communication skills, both written and verbal
• Accurate keyboarding
• PC/Software literate
• Clinical knowledge base sufficient to successfully oversee infusion therapy of complex patients
• Requires completion of task under deadline
• Ability to follow instructions without frequent supervision
EDUCATION AND/OR EXPERIENCE:
• Graduate from an accredited school of nursing. BSN preferred.
• Current work state RN licensure in good standing
• Active driver's license and ability to provide current auto insurance
• Current CPR card
• At least three years' experience as a registered nurse in a home health agency or its equivalent in the past five years.
