About the Nurse Practitioner Position:Key Responsibilities:- Provide care to Subacute Rehab and Long-Term Care residents on our Skilled Nursing Unit and our Personal Care Unit.
- Collaborate with other NPs, Therapists, Nurses and Physicians to provide quality care.
- Examine residents and develop a treatment plan in coordination with other disciplines.
- Order and interpret diagnostic tests.
- Prescribe appropriate medications and therapies.
- Ensure proper documentation in our Electronic Medical Record.
- Ensure that residents and their families are kept apprised of clinical progress during their stay.
Qualifications:- Current and unrestricted Pennsylvania State NP License.
- Current and unrestricted DEA License.
- Desire to work in a subacute rehabilitation, long-term care, and personal care environment.
- Outstanding written and verbal communication skills.
- Ability to document decisions, risks, and treatment plans clearly and effectively.
- Proficiency in using electronic medical records systems.
- Ability to work independently and collaboratively in our healthcare setting.
- Commitment to maintaining up-to-date knowledge through continuing education and professional development.
