We are looking for a compassionate and patient-centered Nurse Practitioner to join our primary care clinic focused on delivering high-quality, value-based care to a senior population. This role is ideal for an NP who is passionate about geriatrics, preventive medicine, and building long-term relationships with patients.
You will work as part of an interdisciplinary care team in a supportive, clinic-based environment designed to allow providers to focus on patient care—not volume. You will report directly to the Associate Medical Director.
Key Responsibilities
- Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
- Manage chronic conditions with an emphasis on preventive care and wellness
- Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
- Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
- Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
- Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in care team meetings and quality improvement initiatives
Qualifications
- Active, unrestricted Nurse Practitioner license in the state of practice
- National certification (AANP or ANCC)
- DEA license (or ability to obtain)
- Experience in primary care, family medicine, internal medicine, or geriatrics preferred
- Comfort working with Medicare and managed care populations
- Strong communication skills and a patient-focused approach to care

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
