Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. As a Physician Assistant or Nurse Practitioner, you'll join a purpose-driven team that:

Your Responsibilities Include:
  • Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
  • Delivering dementia-informed, relationship-based care across the continuum of care.
  • Conducting comprehensive assessments and developing tailored care plans for older adults.
  • Managing chronic conditions and performing care with skill and empathy for patients and their families.
  • Collaborating with cross-functional partners - including palliative, psych, and neuro - to deliver coordinated, whole-person care.
  • Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintaining consistency in documentation, follow-through, and patient communication.
