In this role, you will support patients during their recovery after hospital discharge, helping to reduce readmissions and improve overall outcomes. This position offers a consistent weekday schedule, autonomy, and a collaborative team environment.
Schedule: Monday - Thursday (Daytime Hours)
Key Responsibilities: - Conduct rounds in assigned facilities on a consistent weekday daytime schedule.
- Perform focused patient assessments to monitor recovery, manage pain, wounds, and functional decline.
- Collaborate with physicians, therapists, and facility staff to develop individualized care plans.
- Complete documentation using an electronic health record (EHR) system, with remote flexibility.
- Identify and address readmission risks and gaps in post-acute care.
- Ensure compliance with applicable clinical and regulatory standards.

Qualifications: - Master's degree in Nursing with certification as a Nurse Practitioner (NP) from an accredited program.
- Active, unrestricted NP license with prescriptive authority in the state of practice.
- 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field preferred.
- Experience working in nursing facilities is a plus.
- Strong clinical skills in managing recovery, rehabilitation, pain, and chronic conditions.
- Excellent communication and time-management skills.
