Work Schedule:
Full-time, Monday through Friday, 8:00 am- 5:00 pm. You will work at 1102 S. Park Street in Madison, WI.

As an Advanced Practice Provider, you'll play an integral role in our care teams by providing high-quality and cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to:
  • Conduct psychiatric and AODA assessments of patients in an inpatient and outpatient setting.
  • See patients as needed for new and follow-up psychiatric care, AODA treatment, including medications and psychotherapy.
  • Be responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients with acute and chronic health needs.

Education:
Minimum -
If Nurse Practitioner:
  • Master's or Doctorate's degree from accredited Nursing Program

If Physician Assistant:
  • Bachelor's or Master's degree from accredited PA Program

Licenses and Certifications:
Minimum -
If Nurse Practitioner:
  • Active RN and APNP license in State of Wisconsin AND
  • Active board certification as a NP in Psychiatric-Mental Health.

If Physician Assistant:
  • Active PA license in State of Wisconsin AND
  • Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have:
