Nurse Practitioner or Physician (MD/DO) - Assisted Living Primary Care

$5,000 Retention Bonus | Flexible Full-Time or Part-Time Schedule

Visiting Docs is seeking an experienced Nurse Practitioner or Physician (MD/DO) to provide primary care services to residents in assisted living facilities throughout the Albany Region.

We are looking for a provider who enjoys working with older adults, values autonomy, and is committed to delivering high-quality, patient-centered care.
Schedule Options

Part-Time
  • 2-3 full days per week
  • Travel between Hoosick Falls and Cambridge, NY

OR

Full-Time
  • 4-5 full days per week
  • Travel between Hoosick Falls, Cambridge, and Troy, NY

  • Provide comprehensive primary care services to assisted living residents
  • Assess, diagnose, and treat acute and chronic medical conditions

Required Qualifications
  • Active New York State Nurse Practitioner or Physician license
  • Active DEA registration
  • Currently enrolled and credentialed with both Medicare and Medicaid
  • Ability to practice independently
    • Nurse Practitioners must have a minimum of 3,600 clinical practice hours in accordance with New York State requirements
  • Minimum of 2 years of primary care experience
Preferred Qualifications
  • Geriatric care experience
  • Assisted living, long-term care, or post-acute care experience
  • Strong clinical assessment and communication skills
Important Requirement

Candidates must already be actively enrolled with both Medicare and Medicaid. Due to credentialing timelines, we are unable to consider candidates who are only eligible for enrollment or who would need to begin the enrollment process after hire.
