About the Role

Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you'll provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on what matters most-delivering high-quality, patient-centered care that improves comfort, function, and quality of life.

This role is ideal for a Nurse Practitioner or Physician Assistant who values flexibility, collaboration, and a supportive clinical environment that removes unnecessary administrative burden. You'll benefit from no weekends, holidays, or on-call requirements, a flexible schedule, and a limited patient census that allows you to spend more meaningful time with each individual. With strong back-office support, streamlined credentialing, structured onboarding, and ongoing collaboration with physicians and peers, you'll be set up to focus on clinical decision-making and patient care from day one.

In this role, you'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you'll serve as a key clinical decision-maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.

Location
  • Office Location: Grand Rapids, MI
  • Main Service Area: Grand Rapids and surrounding areas
    • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours
  • Monday to Friday, 8 a.m. to 5 p.m.

Required Qualification
  • Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
  • Current State License as a Nurse Practitioner or Physician Assistant
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage

Preferred Qualifications
  • Minimum 2 years clinical experience in a post-acute or as a Nurse Practitioner or Physician Assistant
  • Adult, Family, or Gerontology certification
  • Experience with geriatric patients

Essential Skills and Competencies
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements and deadlines

Pay Range
  • This role provides a base annual salary and has monthly bonus potential
