Our NP or PA will perform comprehensive health assessment which include but not limited to the following:
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
- Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
- Maintain patient safety and confidentiality
- Universal precautions utilized in all patient care
- Prescribe/order procedures, treatments and medications within scope of practice
- Develop high level of trust and intimacy with patients and families
The Skills, Experience & Education You Bring- CA Licensed Nurse Practitioner or Physician Assistant
- 1+ Years of Experience with Primary Care
The Base Salary for this position is
$140,000 - $155,000/yearIn addition, we provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.
