Department: Specialized Geriatric Clinic

The Nurse Practitioner with the Specialized Geriatric Clinic is responsible for developing collaborative care and treatment plans within respective scope of practice in collaboration with patients, care partners and specialized health team members. The NP will facilitate comprehensive assessment, consultation, system navigation and coordinating of complex care needs in the spirit of optimizing individual functional independence and overall well-being of older adults.

QUALIFICATIONS

Certification/Experience/Knowledge:
  • Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class [RN (EC)] and with a specialty certificate in either Adult or Primary Health Care required
  • Geriatric Assessor Certification or qualification/experience to obtain certification required
  • Demonstrated special interest, knowledge. Applied clinical competence and experience (minimum 5 years) working with a healthcare setting with a focus on older adults required
  • Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (within the last 12 months)
  • Recent clinical experience (within last 2 years) in a mental health outpatient setting preferred
  • Current Canadian Nurses Association (CNA) Psychiatric and Mental Health Nursing certification an asset
  • Experience Motivational Interviewing (MI) Stages of Change, an asset
  • Applied knowledge of key geriatric health assessment tools (e.g. Comprehensive Geriatric Assessment (CGA), Barthel Index, Confusion Assessment Method (CAM), MoCA, Mini Mental Status Examination (MMSE)) in additional to awareness of best practice resources in mental health and behavioural health etc. is required
  • Current PIECES and Gentle Persuasive Approach (GPA) Certificates required

Skills/Abilities:
  • Demonstrated individual and care partner focused approaches supported by best evidence-based practices within respective scope of practice required
  • Dedicated inter-collaborative skills and commitment to ongoing continuing development and quality improvement to facilitate, integrate and spread best practice principles in geriatric medicine, geriatric mental health & behavioural health required
  • Applied knowledge of key geriatric health assessment tools (e.g. Comprehensive Geriatric Assessment (CGA), Barthel Index, Confusion Assessment Method (CAM), MoCA, Mini Mental Status Examination (MMSE)) in additional to awareness of best practice resources in mental health and behavioural health etc. is required
  • Demonstrated skills in management of geriatric syndromes, including but not limited to: delirium, polypharmacy, falls and functional decline, acute/sub-acute cognitive changes, bowel/bladder, and nutrition/hydration conditions required
  • Demonstrated competencies related to safe, effective and ethical prescribing within full scope required
  • Demonstrated skills applying ethical frameworks, conducting comprehensive geriatric assessment, analyzing, diagnostics, developing and administering team-based treatment plans based on the current principles and practices of comprehensive geriatric assessment required
  • Demonstrated knowledge of all specialized geriatric services across the continuum including primary care and community support services required
  • Demonstrated ability to exercise keen professional judgement and critical thinking required
  • Demonstrated strong therapeutic communication skills with the ability to communicate both verbally and in writing in a clear, concise manner, considering the intended audience required
  • Demonstrated ability to manage multiple tasks/projects, diversified workload, rapidly changing priorities and challenging deadlines required
  • Ability to meet the physical demands required within a community based outreach team required
  • Valid driver's license and access to a reliable vehicle to accommodate necessary travel required
  • Must support and contribute to a culture of safety and prevention to ensure the safety of self, fellow workers, patients, volunteers, and visitors required
  • Excellent attendance and work record required
  • Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
