Overview

Geriatric Primary Care NP role in South Chicago. Deliver chronic condition management Monday - Friday (no weekends/eves), seeing patients through 30-45 minute visits. Athena EMR and AI Scribe to get charting done during the work day.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in geriatric primary care managing chronic conditions in alignment to HEDIS/STAR quality outcomes. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

Qualifications

  • Licensed as a NP in the State where HarmonyCares Medical Group office located
  • Federal DEA License/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS Certification
  • Board Certification
  • Must maintain a valid driver's license and good driving record
