Job Summary:

As a Nurse Practitioner, you'll use your clinical expertise to assess, manage, and coordinate the healthcare of chronically ill geriatric participants. You'll work closely with physicians, medical leadership, and clinical operations to ensure safe, accurate, and high-quality care while supporting an interdisciplinary team.

This position is ideal for someone who is analytical, meticulous, and committed to delivering precise clinical results. The environment values disciplined follow-through, critical thinking, and technical excellence.

Schedule:

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:
  • Practice within defined scope and regulatory guidelines, including prescriptive and practice authority.
  • Perform health histories, physical exams, patient education, and follow-up on diagnostic results.
  • Identify urgent or emergent medical conditions and initiate appropriate interventions.
  • Collaborate with physicians and interdisciplinary team members to support optimal participant outcomes.
  • Support clinical operations, including documentation, scheduling, and charting.
  • Participate in quality improvement initiatives and care planning activities.
  • Serve as a clinical resource for staff, participants, and families.
  • Contribute to 24-hour care coordination.
  • Attend meetings and complete other duties as assigned.

Education and Experience:
  • Must be licensed as a Certified Nurse Practitioner in the State of practice.
  • One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.


Requirements:
  • Requires valid State driver's license with a clean driving record.
  • Successful completion of background check, physical, drug screening, TB Screening


Physical Requirements:
  • Must be able to move intermittently throughout the workday.
  • Complete an in-depth orientation to the PACE model of care that may require overnight travel.
  • Is subject to lifting, carrying, and supporting participants.
  • Must be able to drive a motor vehicle.
