Nurse Practitioner

1 FTE Contract to March 31, 2028

Thunder Bay

We are currently seeking 1 FTE contract Nurse Practitioner to join our Homelessness and Addiction Recovery Treatment Hub (HART Hub) who will be responsible for working with an interdisciplinary team to provide a full primary care scope of practice to clients of the HART Hub. There is a strong practice emphasis on disease prevention, health promotion, health education, and community partnerships. HART Hub is a state-of-the-art treatment hub designed to provide comprehensive support and resources that address the root cause of homelessness and addiction.

The position has a competitive salary, the Health Care of Ontario Pension Plan (HOOPP) and an excellent group health benefit package.

The salary range for this position is $112,199 to $134,196 per year. We do not use AI to screen resumes.

Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria

QUALIFICATIONS:

  • BScN in Nursing from a recognized university and in addition a certificate in the extended class from the College of Nurses of Ontario (CNO).
  • Thorough knowledge and proficiency in current extended class nursing procedures, physical assessment and treatment measures within the RN-EC scope of practice.
  • Experience in a community-based primary health care setting, addictions, homelessness, and/or mental health is an asset.
  • Strong understanding and alignment with the concepts of trauma-informed care and culturally safer care.
  • Ability to navigate complex case management situations, advocate for clients effectively, and create individualized care plans.
