Responsibilities:
Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists.
Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices.
Monitor patient progress and adjust treatment plans as required.
Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care.
Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
Participate in quality improvement initiatives and support best practices in patient care.
Adhere to all regulatory, professional, and organizational standard
Perform consultations with patients as assigned.
Perform stress test supervision as assigned.
Perform all medical tasks delegated by the attending physician.
Contribute to ensuring there is a safe environment for patients, staff and visitors.
Maintain safe and clean working environment by complying with procedures, rules, and regulations.
Call for assistance from health care support personnel, if required.
Document patient care services by charting in patient records.
Take all patient vitals and history.
Maintain patient information electronically.
Document and record medicine and prescriptions.
Perform any and all other tasks as assigned by the supervisor.
Qualifications, Experience & Requirements:
0-2 years of related experience.
Nurse Practitioner certification is required.
Member in good standing with the College of Nurses of Ontario.
