Job DescriptionTitle - Nurse Practitioner Manager, Palliative Care at Home - Mount Sinai Home Health - Full Time - DayPalliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients' healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education.
Responsibilities• Responsible for clinical and operational oversight.
• Develops strategies to drive service line growth, clinical excellence, and attract and retain staff in collaboration with leadership.
• Collaborates with leadership to create financial strategies that align with the organization's goals and objectives.
• Serves as an expert and resource in symptom management, demonstrating sound clinical judgment and prescribing proficiency as a nurse practitioner/clinical leader.
• Facilitates family meetings and goals of care conversations.
• Responsible for the onboarding, orientation, and ongoing clinical education of the team.
• Promotes a team environment based on company mission, vision, and values. Acts as a clinical resource for the team and collaborates with the lead physician.
• Communicates organizational policies and procedures, evidence-based best practices, and
regulatory/industry changes to direct reports.
• Holds employees accountable for the attainment of defined goals/objectives and provides performance redirection when needed.
• Ensures compliance with policies, procedures and regulatory mandates.
• Performs all other duties as assigned.
Qualifications• Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate.
• Licensure: New York State License with current registration as a registered nurse.
• State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows:
• a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination.
• b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).
• c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner.
OR
• Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially)
- Three or more years of clinical experience in palliative care/geriatrics as a nurse practitioner.
- Two or more years of progressive leadership including team-building and influencing growth.
- Licensed as a Nurse Practitioner in the state of practice.
- National certification in a primary field (e.g. Adult, Family, Gerontology, Acute Adult, and Acute preferred)
- Hospice and Palliative Certification (e.g. Advanced Certified Hospice and Palliative Nurse (ACHPN) from the Hospice and Palliative Credentialing Center (HPCC)).
- BCLS and/or ACLS certification. Issuing Agency: AHA
- Drug Enforcement Administration (DEA) certificate required.
- Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges
Compensation StatementThe Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $115,494.00 - $192,328.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
