Responsibilities

  • Perform physical exams and preventive health measures as per physician's guidance.
  • Order, interpret, and evaluate diagnostic tests to assess patients' clinical problems.
  • Record physical findings and formulate prognosis based on patient conditions.
  • Discuss cases with Physician and health professionals to develop patient care plans.
  • Submit health care plans and goals for review by Physician and evaluate outcomes.
  • Prescribe or recommend treatments and refer patients to specialty care as needed.
  • Travel occasionally for home visits to provide care.

Full Job Description
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician. Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. Records physical findings, and formulates plan and prognosis, based on patient's condition.
  • Discusses case with Physician and other health professionals to prepare comprehensive patient care plan.
  • Submits health care plan and goals of individual patients for periodic review and evaluation by Physician.
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to Physician for consultation or to specialized health resources for treatment. The position requires occasional travel and home visits.


EDUCATION AND/OR EXPERIENCE

Graduate of an accredited nursing program.
Graduate of an accredited masters of nursing program.
Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.

CERTIFICATION/LICENSURE

Hold valid RN license in state of practice.
Hold valid APNP license in state of practice.

OTHER SKILLS AND ABILITIES
Excellent interpersonal and communication skills.
Analytical and interpretive skills.

LEVEL OF SUPERVISION

Indirectly supervises other palliative care nursing or support staff.

$85,280.00 - $144,976.00
