Overview

A Nurse Practitioner, certified under section 6910 of the New York State Education Law is a licensed registered nurse who provides primary health care in accordance with written policies approved by nursing/medical administration. Responsible for patient care as outlined within the Collaborative Practice Agreement.

NURSE PRACTITIONER (NYSNA)

NURSE PRACTITIONER - GERIATRIC HOME VISITS

Monday-Thursday: home visits 9am-5pm

Friday: admin time 9am-1pm, MDP clinic 1pm-5pm

Responsibilities

      • For direct care providers: Provides age-appropriate care and intervention for ages of patients served according to job description (e.g. assessments, treatment plans, assists with assessments).


      • For non-direct care providers: Provides support services using age appropriate
      • interpersonal communication.


      • Clinical Practice Domain
      • Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative practice agreement. See Attachment D.


      • Assesses the health status of clients/families by means of comprehensive health history and physical examinations.


      • Initiates therapeutic and diagnostic procedures, consultations and laboratory examinations. Interprets findings and alters plan of care including, but not limited to, prescribing and altering medication regimens and ordering blood according to agreed-upon collaborative practice agreement and designated protocols in designated area.


      • Provides teaching and counseling in areas of health promotion and maintenance and in disease prevention


Documents findings and plan of care in patient's medical record

      • Consults with physician as established in the collaborative practice agreement.


      • Initiates referrals to specialty services and fosters continuity of care.


      • Initiates hospital admission and discharge after collaborative consultation with physician.


      • Works collaboratively with interdisciplinary team to promote optimal patient
      • outcomes.


      • Assessment, treatment and patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.

      • Demonstrates accountability for nursing practice.


      • Advocates for patients and supports patient rights.


      • Serves as a resource person to nurses and other health team members.


      • Participates in peer review.


      • Participates in approved medical and nursing research studies.

        determine policies affecting nursing practice.


      • Collaborative Physician Agreement


      • Performs duties and responsibilities in accordance with agreement between Nurse Practitioner and collaborating physician with endorsement by Vice President, Hospital Operations and Chairman of the department.


      • Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.

OTHER JOB DUTIES
  • Keeps knowledge current by self-directed learning and attendance at educational programs.
  • Performs other related duties as required.

Qualifications

Requirements:
  • Master's or Doctorate in Nursing
  • Current and unrestricted Nurse Practitioner license in New York
  • National certification as an Adult-Gerontology Primary Care NP (AGPCNP) or Family NP (FNP)
  • Valid driver's license and reliable transportation
  • Minimum of 2 years' experience in geriatrics, internal medicine, or home-based care
  • Excellent clinical, communication, and critical thinking skills
  • Comfortable working independently in diverse home environments
  • 3-5 years' experience as an NP
  • Able to practice independently without collaboration
  • Experience with home-based primary care or home visits


Preferred Qualifications:
  • Knowledge of chronic care management, dementia, falls prevention, and polypharmacy
  • Familiarity with comprehensive geriatric assessments
  • Familiarity with EMR systems and mobile documentation
  • Experience with program management and development (e.g., expanding a home visit practice)

Pay Range

USD $151,740.55 - USD $151,740.55 /Yr.
