Qualifications

• Baccalaureate of Science in Nursing; Master of Science in Nursing required
• Registered with the College of Nurses of Ontario as RN(EC)
• Current Membership with NPAO
• Displayed liability insurance
• Valid Ontario Driver's License
• Experience in Community and/or Primary Care Setting
• Experience working with vulnerable populations in mental health and addictions
• Proficient in evidence-based primary care delivery methods
• Strong organizational, evaluation, and time management skills
• Familiar with Microsoft Office software; experience with electronic medical records preferred
• Eagerness to enhance skills through continuous learning
• Effective multi-tasking and teaching ability while treating patients
• Excellent verbal and written communication skills
• Non-judgmental and open-minded demeanor

• Provide comprehensive primary care services across all life stages
• Emphasize health promotion, patient education, and disease prevention
• Develop, implement, and monitor patient programs and services
• Collaborate with the interprofessional team to deliver integrated care
• Evaluate success and outcomes of health initiatives
• Engage with families and communities for holistic care
• Maintain up-to-date knowledge in best practices for primary care delivery
