Full-Time Nurse Practitioner

The Charles B. Wang Community Health Center, anaward-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We arelooking for team-oriented individuals interested in delivering high qualityprimary care in a community health setting. Our collegial outpatient-onlypractice offers strong ancillary support, including nursing support, referralcoordinators, health educators, on-site mental health services, and socialworkers.

Responsibilities:

  • Involve with direct patient physical assessment, education, and health planning
  • Maintain continuity of care by regular patient follow-up
  • Assist in the coordination of services with other disciplines and patients' insurance plans
  • Advocate for patient in arrangement of medically necessary services
  • Communicate with community resources to ascertain adequate assistance for patients in need
  • Refer patients to specialty services when necessary
  • Consult with collaborating physician and other members of the health care team to establish plan of care
  • Participate in quality assurance process to assure optimal care delivery
  • Provide comprehensive case management services that include emphasis on health promotion and disease prevention
  • Monitor patient outcome in accordance to the practice protocols
  • Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
  • Act as a resource person for the nursing staff
  • Conduct education in service on practice issues
  • Perform other duties as assigned


Minimum Qualifications:
  • Graduated from approved Nurse Practitioner Program
  • Licensed as Nurse Practitioner in New York State
  • Basic CPR (BCLS) certification required
  • At least 3 years of clinical experience as a primary care Nurse Practitioner
  • Board/Specialty Certification preferred

Compensation and Benefits:
  • Full-time annual base salary at $105,000 - $115,000, with additional productivity-based incentive pay
