The Nurse Practitioner applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.
This is a full time NP/PA (APP) role.
Multistate licensure preferred in TX, AZ, VA, FL, MS, GA. This is a fully remote position.Responsibilities:
- Call Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months)
- Create and register a patient record within Athena
- You will verify benefits
- You will gather medication, medical and surgical history
- Coordinate management of members social needs to help facilitate gap closure - providing transportation services.

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
