Qualifications

  • Board Certified or Board Eligible in Family, Internal, or Geriatric Medicine.
  • Minimum of 2 years experience as a Nurse Practitioner in primary care.
  • Fluent in both Spanish and English due to patient demographics.
  • Must reside within 15-30 minutes of the clinic.

Responsibilities

  • Provide outpatient care with a focus on Medicare Advantage patients.
  • Assess and manage a daily patient load of 12-18.
  • Collaborate with a multi-disciplinary team to enhance patient outcomes.
  • Maintain comprehensive and accurate patient records.
  • Educate patients on health management and disease prevention.

Job Description:
  • 100% Outpatient - NO CALL
  • Monday through Friday 8am-5pm
  • Patient Population: Medicare Advantage and fluent Spanish speaking
  • 12-18 patients daily


Education & Experience
  • MSN
  • Board Certified or Board Eligible in Family, Internal, or Geriatric Medicine

  • Fluent Spanish and fluent English required due to patient demographics
  • Must live within 15-30 minutes from clinic
