Position Title: Nurse Practitioner

Reports To: Senior Director of Compliance & Clinical Operations

Location: Monday - Friday | 9:00 AM - 5:00 PM | Home Base: Brooklyn, NY, 11218

Job Summary: The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results.
  • Diagnosis and Treatment: Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
  • Preventive Care: Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
  • Health Education: Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
  • Care Coordination: Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.
  • Telehealth: Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.
  • Record Keeping: Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.
  • Prescription Management: Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.
  • Compliance: Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.
  • Community Health: Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.
  • Follow-ups: Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.


Requirements:

  • Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.
  • Licensure: Possess a valid and unrestricted nurse practitioner license in the State of New York.
  • Certification: Be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Clinical Skills: Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans.
  • Communication Skills: Have excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.
  • Bilingual in Spanish is required.
