Primary Care

Location: Aurora, IL

The Role

This position is designed for a Nurse Practitioner (NP) or Physician Assistant (PA) who wants to practice Outpatient Clinical Care in a supportive, collaborative environment. You will be fully integrated into the physician's practice where you will:

  • Work alongside a lead physician to co-manage a defined panel of geriatric patients
  • Collaborate with a team of dedicated healthcare professionals, providing high-quality care to patients in an outpatient clinic setting.
  • Perform preventative screenings and lead important discussions regarding long-term goals of care.
  • Review and update patient medication lists to ensure accuracy, and order appropriate laboratory and diagnostic procedures
  • Maintain and review patient records, charts, and other pertinent information accurately and efficiently.
  • Maintain a high level of independent clinical decision-making in a fast-paced environment.
  • Ensure patients receive care across the entire continuum and provide secondary support within various post-acute settings such as AL/IL and SNFs.
