The RoleThis position is designed for a Nurse Practitioner (NP) or Physician Assistant (PA) who wants to practice
Outpatient Clinical Care in a supportive, collaborative environment. You will be fully integrated into the physician's practice where you will:
- Work alongside a lead physician to co-manage a defined panel of geriatric patients
- Collaborate with a team of dedicated healthcare professionals, providing high-quality care to patients in an outpatient clinic setting.
- Perform preventative screenings and lead important discussions regarding long-term goals of care.
- Review and update patient medication lists to ensure accuracy, and order appropriate laboratory and diagnostic procedures
- Maintain and review patient records, charts, and other pertinent information accurately and efficiently.
- Maintain a high level of independent clinical decision-making in a fast-paced environment.
- Ensure patients receive care across the entire continuum and provide secondary support within various post-acute settings such as AL/IL and SNFs.

Ideal Candidate- Board-certified NP or PA
- Interest in geriatrics (experience preferred but not required)
- Comfortable in multiple care settings
- Strong communicator with patients and families
- Detail-oriented and thoughtful clinician
- Values mentorship and team-based practice
- New graduates with strong clinical rotations in geriatrics may be considered.
Schedule & Structure- Full-time
- Mix of clinic days and facility rounding days
