🩺 What You'll DoAs a Primary Care Provider, you will:
- Deliver comprehensive primary care services across the lifespan.
- Perform preventive care, wellness visits, and chronic disease management.
- Diagnose and treat acute and chronic medical conditions.
- Independently manage an established patient panel.
- Coordinate care with specialists and community resources.
- Collaborate closely with behavioral health, care management, and support teams.
- Participate in quality improvement and population health initiatives.
- Document patient care accurately within the electronic health record.

• Required Experience & QualificationsNurse Practitioner (APRN)- Current New Hampshire APRN and RN licensure.
- Board Certification through ANCC and/or AANP.
- Current DEA registration.
- Current BLS certification.
Physician Associate (PA)- Current New Hampshire Physician Associate license.
- NCCPA Certification.
- Current DEA registration.
- Current BLS certification.
Experience- Candidates must have a minimum of one (1) year of independent outpatient clinical practice experience. Providers with three to five (3-5) years of independent primary care experience are strongly encouraged to apply.
- Demonstrated experience independently managing an established primary care patient panel.
- Experience providing comprehensive primary care services across the lifespan.
- Experience diagnosing and managing acute and chronic medical conditions across the lifespan.
- Primary Care, Family Medicine, Internal Medicine, Community Health, Urgent Care, or FQHC experience is strongly preferred.
- Strong commitment to team-based care, quality outcomes, and population health management.
