Job Summary:We're currently seeking a Nurse Practitioner to join our outpatient Primary Care team-an opportunity to practice in a supportive environment that prioritizes professional growth, innovation, and physician well-being.
Key Position Details:The Faribault Clinic is searching for a Nurse Practitioner to join its team. Here you will be part of a team including Primary Care Providers and the service line specialties of Eye, Ortho, Podiatry, Allergy, Cardiology, Weigh Mgmt, Derm, ENT, Gen Surg, MH, OB/GYN, and additional ancillary services yet able to access the full spectrum of specialty care across the Allina Health system.
- Flexible FTE
- Primary care team has 4 Family Medicine Physicians and 4 APPs
- 36 patient contact hours per week
- Minimal call coverage, phone only, RN triage support
- Outpatient practice providing care to all ages, including growing families
- Clinic hours are Monday -Friday: 7:00am-5pm, and closed Saturday-Sunday

Job Description:Provides help with all aspects of patient care, including diagnosis, treatments and consultations as part of a physician/nurse team in accordance with the designated specialty. Works in both inpatient and outpatient situations and can perform independently or as part of a treatment team. Serves as an educator, case manager, consultant to staff and multidisciplinary health care team.
Principle Responsibilities- Provides clinical care for patients
- Completes comprehensive patient assessments.
- Determines diagnosis and treatment of illness.
- Orders and interprets medical diagnostics in tandem with attending physician.
- Prescribes medications, treatments, therapy and medical equipment.
- Conducts patient education when working to manage chronic illness or wellness care.
- Refers for specialty care when determined.
- Consults with physician when patient needs outside of Nurse Practitioner scope of practice, diagnosis or treatment plan is unclear.
- Participates in health care team
- Promotes communication within the team and other interdisciplinary health care staff.
- Acts as resource to members of the team.
- Works with primary care team for program development.
- Acts as liaison with outside agencies as needed.
- Other duties as assigned.
Required Qualifications- Master's degree completion of accredited Nurse Practitioner program
Preferred Qualifications - Minimum 2 years experience post Nurse Practitioner training strongly preferred
Licenses/Certifications- Licensed Registered Nurse - MN Board of Nursing required upon hire
- Certified Nurse Practitioner - ANCC - American Nurse Credentialing Center (ANCC) required certification within specialty upon hire
- ACLS-BLS Tier 2 - Multisource required within 180 Days
- Drug Enforcement Administration required upon hire
- Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment for those working in Complex Care for Seniors, Community Palliative Care and Hospice
Physical Demands- Medium Work*:
- Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), up to 25 lbs. frequently
Pay RangePay Range: $279,825 to $329,175 per year
The pay described reflects the starting base pay range, assuming a full-time FTE. Your starting rate will depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
