- The Nurse Navigator is responsible for enhancing the communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs.
- Assists in the development of strategies to reduce LOS and re-admissions.
- Part of the cardiovascular team which monitors LOS and re-admission data.
- Monitors care process and develops approach to improve care efficiency and patient outcomes.
- Performs other related duties as assigned.

QualificationsEducation- Bachelor's/Master's in Nursing or related field
License/Certification- Current NJ (New Jersey) Registered Nurse License required
- BLS required
Relevant Experience- 3 years of cardiology nursing
