Qualifications

• Current Illinois LPN or RN license required
• 2+ years of nursing experience in relevant fields preferred
• Prior leadership or care coordination experience preferred
• Strong clinical assessment and critical thinking skills
• Experience in managing complex care transitions
• Ability to manage multiple priorities independently
• Commitment to compassionate, resident-centered care

Responsibilities

• Serve as the primary clinical coordinator for residents
• Oversee and manage clinical care plans for recovery
• Collaborate with interdisciplinary team members
• Monitor resident progress and address condition changes
• Educate residents, families, and staff on care goals
• Support admissions and care coordination for rehabilitation residents
• Ensure compliance with relevant regulations and policies
• Participate in weekend on-call rotation
