Skilled Nursing Facility to provide assistance and care for our wonderful residents. You will administer & document medications, assist in admission & discharge procedures, communicate residents' conditions to physicians and family members, and more.

Responsibilities:- Administer medication and treatments to residents
- Respond to resident's emergency calls in a timely manner
- Complete wound treatments when necessary
- Assure residents are being cared for per established guidelines
- Responsible for following up on concerns reported by non-licensed staff
- Maintains accurate documentation of medication administered and care rendered.
Requirements:- Active RN/LPN License in Illinois required
- A desire to provide a high level of care and customer service is required
- 1 year of experience working in a skilled/rehab setting required
- Excellent organization skills
