Job Details

This role is in support of Cencora's patient and provider support and pharma commercialization services in Canada marketed through our Innomar Strategies business.

  • Plans and manages complex care coordination cases and activities to support timely patient access, treatment progression, and continuity of care.
  • Conducts assessments, supports care planning, and facilitates transitions of care that address clinical, social, and access barriers affecting patient outcomes.
  • Coordinates with providers, pharmacies, payers, and internal partners to resolve access barriers, align next steps, and maintain case continuity.
  • Analyzes case trends, recurring issues, and workflow patterns to identify root causes and recommend process improvements.
  • Develops and refines job aids, care coordination templates, and documentation standards to improve consistency and reduce rework.
  • Guides less experienced team members on complex case handling, escalation practices, and documentation expectations.


Education:
  • Bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related field, or equivalent experience required.


Work Experience:
  • 4+ years of experience in care management, case management, clinical support, patient services, or a related field required.


Certifications:
  • Certification in case management, care coordination, clinical specialty, or equivalent preferred.
