Plans and manages complex care coordination cases and activities to support timely patient access, treatment progression, and continuity of care.

Conducts assessments, supports care planning, and facilitates transitions of care that address clinical, social, and access barriers affecting patient outcomes.

Coordinates with providers, pharmacies, payers, and internal partners to resolve access barriers, align next steps, and maintain case continuity.

Analyzes case trends, recurring issues, and workflow patterns to identify root causes and recommend process improvements.

Develops and refines job aids, care coordination templates, and documentation standards to improve consistency and reduce rework.

Guides less experienced team members on complex case handling, escalation practices, and documentation expectations.

Bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related field, or equivalent experience required.

4+ years of experience in care management, case management, clinical support, patient services, or a related field required.

Certification in case management, care coordination, clinical specialty, or equivalent preferred.
