Full Job Description
Job Summary:
This position will play a key role in supporting the newly established Lyme Disease Center by providing specialized care to patients affected by Lyme disease and related illnesses. The candidate will help advance the Center's mission by offering expert clinical management, contributing to patient education, and collaborating on innovative treatment approaches. Previous experience in ID, procedural and or infusion services is preferable.

Responsibilities include: Collaboration with the Attending Physician and a multidisciplinary team, provide comprehensive health assessments and develop a plan of care for a diverse and complex patient population, supporting a robust ambulatory clinic including infusion services. Document in the medical record by recording subjective, objective data, assessment and plan, record data in a consistent, legible, accurate, concise and timely manner. Will provide patient/family education and utilize prescriptive authority working in collaboration with the attending physician. Candidates should be compassionate, friendly, strong clinical, critical thinking, assessment skills, communication skills and enjoy a collaborative setting.
Minimum Qualifications:
NYS licensed/registered NP or PA required (NYS NP specialty area of certification must be consistent with practice). Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree) (eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA). Initial certification must be present or obtained within 6 months of hire.

Current BLS certification through the American Heart Association or the American Red Cross is required.
