NP/PA Annual Salary: $157,684.80 to $192,712.00 (Based on a 40 hour work week)

LOCATION:

Mobile Health Unit within Napa county and few shifts at South Napa Campus (off Hartle Court)

JOB SUMMARY/OVERVIEW:

The Mobile Health Provider will be responsible for providing quality primary health care to patients in mobile health settings as well as on-site scheduled shifts in health centers. They will travel extensively with the mobile health team providing care to the community at large, including, but not limited to, farmworkers, unhoused, students, seniors, and other vulnerable populations. They will be working closely with other healthcare staff in a team approach. As a clinical position, they will also provide clinical services to patients in accordance with their experience and training. MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
  1. Education:

1. NP or PA-C with at least 3 years of experience preferred.

3. Graduate of an accredited Master of Science in Nursing program required, or graduate of a Physician Assistant Program from an accredited school.

4. Current Family Nurse Practitioner certification required (for FNP position).

5. Current Physician Assistant License required and National Commission on the Certification of Physician Assistants Certificate required for PA applicants

6. Current California Provider certification/licensing required.

7. Must have or obtain current California medication furnishing license.

8. Must have or obtain current DEA License.

1. Experience in a Federally Qualified Health Center preferred

2. Experience with mobile health or street medicine teams preferred
  1. Special Skills/Training:

Must certify and remain current in CPR certification.

2. Strong verbal and written communication skills; Bilingual (English/Spanish) language strongly preferred.

3. Excellent interpersonal and customer service skills required; must have ability to treat patients with complex medical and social needs.

4. Must have strong time management skills, ability to prioritize, and work in a fast-paced environment.

5. Computer and data entry skills required; electronic health record system preferred.

6. Excellent problem-solving skills required.

7. Ability to work autonomously with a mobile health team away from a health center.

8. Drivers License Required.

9. Flexibility to work in a variety of environments.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1.

Travel extensively to community locations and other outreach destinations with the mobile health team multiple days per week -including evenings, and weekends as needed. Potential to provide care within health center should Mobile Health not be active (example, but not limited to weather, vehicle maintenance, unforeseen reasons) and community health screening events and supporting community health events.

2.

Provide the following health care services: assessment/evaluation, diagnosis, treatment/prescribing, planning and education. Consult with staff physicians, pharmacists or other care team members as needed.

3.

Review and manage patient information.

4.

Prescribe and administer medications and treatment regimens according to assessed medical conditions. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions.

5.

Review and manage patient information including labs, diagnostic images, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc. - per protocol.

6.

Refer patients to specialists and to relevant patient care services as appropriate.

7.

Perform various outpatient procedures as indicated, appropriate, or per privileging.

8.

Prioritize patient care throughout the day.

9.

Provide acute triage as needed and lead direction of the shift flow during field shifts.

10.

Provide education and information to patients on various diseases, providing education material as appropriate.

11.

Perform various administrative procedures as directed and complete various medical related forms as needed.

12.

Document all appropriate information in electronic health record system.

13.

Respond to messages and relay information to patients and other external sources as needed.

14.

Direct and coordinate the patient care activities of support staff as appropriate.

15.

May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.

16.

Assists in the coordination and integration of health clinic outreach programs as directed.

17.

May be required to respond to afterhours outpatient triage calls on a rotation basis.

18.

Works collaboratively and effectively within a team.

19.

Provides excellent customer service and treats everyone with dignity and respect. Seeks to assist community members; responds to requests for service and assistance; manages difficult or emotional patient situations; responds promptly to patient needs.

May be required to participate in the education of students, community health workers, or others in an education/learning situation.
