ABOUT THE ROLE

We are seeking a Market Engagement Manager to serve as the "quarterback" for our Nevada operations. You will be the face of Tembo Health for primary care practices, specialists, and community partners across the state. Your mission is to ensure that every patient and caregiver facing dementia has a seamless pathway to the expert care management and navigation they deserve.

This is a hybrid role that requires strategic relationship building, data-driven practice management, and a "roll-up-your-sleeves" approach to market operations and growth.

RESPONSIBILITIES

1. Practice Engagement & Growth
  • Relationship Management: Build and maintain deep, long-term partnerships with key stakeholders, primary care practices, affiliate clinicians, and key specialists.
  • Referral Optimization: Work on-site and virtually with clinicians, practice managers, and clinical teams to implement smooth EMR referral workflows, ensuring Tembo is the "easy button" for dementia support.
  • VBC Performance Support: Support network and clinician efforts related to clinical performance to influence value-based care outcomes, including total cost of care, utilization reduction (ER/Inpatient), and quality improvement processes.
  • Data-Driven Insight: Present patient-level information and clinical insights to practices to help them identify and refer eligible families for Tembo's dementia care services, and discuss ongoing patients in Tembo's dementia care program.
  • Performance Tracking: Monitor market-specific KPIs, ensuring high clinician satisfaction and low referral friction.

2. Market Development
  • Network Building: Beyond primary care, you will establish Tembo's presence with specialty clinics (e.g., Neurology, Geriatrics, Psychiatry), community resource centers, home health agencies, and senior advocacy groups focused on home-based care.
  • Market Initiatives: Execute local "road shows," socialization events, and marketing initiatives to drive brand awareness and referral volume.

3. Operational Excellence
  • Project Management: Own the Nevada rollout project plan, ensuring all tactical milestones are met on time.
  • Feedback Loop: Serve as the bridge between Nevada practices and our internal Clinical and Operations teams to improve our tools and workflows.

4. Patient Success & Care Navigation (Bonus Opportunity)

While the primary focus of this role is growing the market and establishing referral relationships, there is an opportunity to directly impact the patient journey.
  • Care Coaching: For a subset of patients in your market, you may provide monthly care navigation-serving as their dedicated point of contact to coordinate care, answer questions, and ensure they are utilizing their full range of benefits.
ABOUT YOU
  • Experience: 5+ years in healthcare or managed care (Provider Relations, Population Health, Practice Management, or Customer Success).
  • VBC Knowledge: Deep understanding of Value-Based Care, Medicare Advantage, and the challenges facing networks and primary care practices today.
  • Entrepreneurial Spirit: You thrive in a fast-paced "startup" environment. You don't wait for a playbook; you help write it.
  • Communication: Exceptional presentation skills. You can speak the language of an Executive, a Physician, and a Family Caregiver with equal ease.
  • Technical Savvy: Comfortable with EMRs (Athena is a plus), CRM tools, and data analysis to drive decision-making.
  • Location: Must be based in Nevada (Clark County preferred) with the ability to travel locally to practices.
SALARY & BENEFITS
  • Salary Range: $70,000 - $100,000 (commensurate with experience) + Performance Bonus + Equity.
