Overview

The Manager of Partner Engagement and Performance Improvement works closely with assigned partner organizations, facilitating their success if value-based care initiatives. This role is responsible for the engagement and performance of provider partners; the development and implementation of value-based payment, quality improvement, and other population health strategies; monitoring of partner performance metrics and key performance metrics related to implemented population health strategies; and coaching partners on improving performance.

The Manager may also supervise Program Associates or Program Coordinators on the team.

Responsibilities

  • Partner Engagement:
    1. Cultivate strong relationships with assigned partner organizations, serving as the primary point of contact for communication and collaboration.
    2. Collaborate with internal teams to understand the unique needs of each partner organization and tailor engagement strategies accordingly.
    3. Participate in meetings at partner sites and follow up on action items from those meetings.
    4. Continuously assess level of partner engagement and develop remediation plans when engagement is low.
  • Performance Data Monitoring and Sharing:
    1. Regularly monitor and analyze performance data for partner organizations, providing actionable insights to drive improvement in key performance indicators.
    2. Develop and implement effective communication strategies to share performance data with partners, fostering transparency and accountability.
    3. Support data retrieval and reporting from partner sites.
  • Onboarding and Training:
    1. Lead the onboarding process for new partner organizations, ensuring a smooth transition into value-based care and population health initiatives.
    2. Develop and deliver training programs to enhance the value based care capabilities of partners.
  • Resource Connection:
    1. Identify and connect practices with relevant resources, tools, and support services to enhance their ability to deliver value-based care.
    2. Collaborate with internal and external partners to ensure practices have access to the necessary resources for success.
  • Implementation of Population Health and Performance Improvement Strategies:
    1. Work collaboratively with practices to design and implement performance improvement strategies aligned with value-based care goals.
    2. Provide ongoing support and coaching to practices in executing and sustaining effective improvement initiatives.
    3. Collaborate with internal teams to design and implement targeted approaches and programs to support value-based care and population health goals
  • Report and Material Development:
    1. Participate in the development of reports, presentations, training collateral, and other materials in support of team activities and initiatives.
    2. Ensure that communication materials are accurate, compelling, and aligned with organizational messaging.
  • Supervision of Team Members:
    1. Supervise and provide leadership to program associates and coordinators within the team.
    2. Foster a collaborative and supportive team environment, ensuring effective delegation and coordination of responsibilities.

Qualifications

Education:

Master's degree in public health, public administration, health services administration, business, or related fields

Experience:
  • Minimum 4 years of experience in the healthcare sector. Experience with quality improvement and ambulatory care services preferred.
  • Minimum of 2 years of project management experience.
  • Experience with implementing change efforts within healthcare organizations.
  • Experience with health IT and electronic medical records preferred.

Skills:
  • Proficiency in establishing and maintaining key relationships.
  • Excellent interpersonal, problem-solving, decision-making and judgment skills.
  • Managerial and supervisory skills.
  • Advanced skills in Microsoft Office and technology systems.
  • Comfortable with data analysis and interpretation.
  • Excellent written and verbal communication skills.
  • Comfortable with public speaking and presenting.
  • Strong project management and organizational skills.
  • Knowledge of Value-Based Care and Value-Based Programs (VBC/VBP), Patient-Centered Medical Home standards or Advanced Primary Care (APC) standards, Accountable Care Organizations, physician incentive programs, and other healthcare reform efforts.

Pay Range

USD $80,000.00 - USD $90,000.00 /Yr.
