As a Manager of Care Navigation, you will:
• Support departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate
• Lead team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner
• Ensure active participation in Interdisciplinary Rounds and other patient care conferences
• Lead delivery of care provided by Care Navigation team to ensure that patients' needs are addressed, and that documentation meets standards
• Implement the departmental strategic plan
• Collaborate with Utilization Review leadership to understand trends of discharge barriers and denials
• Facilitate the development of systems, processes and PI that drives optimal performance for the department
• Oversee and evaluate the performance of the Care Navigation team

Qualifications include:
• RN: Bachelor of Science in nursing required, Master's degree preferred
• SW: Master of Social Work required
• 3-5 years of experience in patient navigation, discharge planning, or care coordination
• At least one year of prior leadership experience required
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
• Maryland RN, LCSW-C, or LCSW licensure with plans to achieve LCSW-C in 1 year
• Active American Heart Association Basic Life Support (BLS) certification required

Pay Range:
$89,356.80 - $134,035.20
If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
