Job Summary and Responsibilities
As our Manager of Care Coordination, you will help the Care Coordination department and your interdisciplinary team members manage daily operations and patient progression so they can improve patient outcomes, operational efficiency, and the overall quality of care.
Every day you will oversee department workflows, supervise staff, and monitor performance metrics to ensure seamless service delivery. You will be expected to partner with physicians and leadership to resolve barriers to patient care, enforce regulatory compliance, and foster a collaborative, high-performing team culture.
To be successful in this role, you will possess strong analytical and critical thinking skills, combined with the ability to lead, mentor, and delegate effectively within a complex healthcare environment.
- Provides operational leadership for the Care Coordination department through staff supervision, team development, coaching, and performance management.
- Coordinates and implements Care Coordination programs, initiatives, and workflows to support patient progression, resource utilization, quality outcomes, and organizational goals.
- Manages daily department operations, including staffing assignments, workload balancing, scheduling, productivity, and resource allocation.
- Supervises front-line Care Coordination staff, including case managers, social workers, and support personnel, ensuring accountability for performance and service excellence.
- Ensures timely completion of patient assessments, discharge planning, utilization management collaboration, documentation, and care transition activities in accordance with organizational standards.
$5,000 Sign on for eligible new hires with at least 1 year of relevant experience within the last 3-5 years.
Job Requirements
Required:
- Bachelor's Degree in Nursing (e.g. BSN) or Masters Degree in Social Work or equivalent education and experience
- Minimum of three (3) years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or three (3) years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination and discharge planning
- Operational experience, implementation, and staff development in healthcare environment
- Current Registered Nurse (RN) license in California. (No License required for Masters Social Worker)
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) required within 1 year of hire.

- Apply clinical guidelines to ensure progression of care
- Communicate effectively in writing and verbally
- Must have analytical, critical thinking and problem-solving skills
- Collaborate effectively with multiple stakeholders
- Proficient in the use of computers and multiple software programs
- Actively work within electronic health record (Epic, Cerner, Meditech, and/or Paragon)
- Understand how utilization management and care coordination programs integrate
- Work as a team player and assist other members of the team where needed
- Thrive in a busy, self-directed environment
- Knowledge of CMS standards and requirements
- Proficient in prioritizing work and delegating where indicated
- Highly organized with excellent time management skills
Preferred:
- Master's degree in Nursing, Health Care Administration, or related clinical field
- Experience with performance improvement activities
- Licensed Clinical Social Worker (LCSW)
