PURPOSE AND SCOPE:

The Manager, Kidney Care Specialist provides strategic leadership and operational oversight for a team of Kidney Care Specialist who support individuals living with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) across both field-based and virtual settings. This leader ensures high-quality patient engagement, consistent execution of education and navigation workflows, and seamless coordination across physician practices, communities and virtual care pathways.

This role translates enterprise strategy into regional execution, accelerating market adoption of early CKD education, strengthening referral pipelines, and improving patient readiness prior to dialysis initiation. The Manager partners with clinical leaders, physicians, operations, market development, and cross-functional stakeholders to align workflows, optimize performance, and continuously improve both field and virtual patient-support programs in alignment with Fresenius Kidney Care's clinical, quality, growth, and value-based care objectives.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Leadership, Coaching & Team Development
• Directly lead, coach, and develop a team of Kidney Care Specialist
• Set clear performance expectations aligned to referral growth, education engagement, and completion outcomes
• Conduct regular individual one on one's, check-ins, team meetings, field ride along, virtual session observation, and performance reviews.
• Support recruiting, hiring, onboarding, and ongoing competency development.
• Monitor workloads, caseloads, and territory coverage to ensure balanced, high quality patient support.
• Cultivate a high performing team culture rooted in patient-centered care, accountability, and continuous improvement.

Program Execution: Navigation, Education & Engagement
• Oversee delivery of navigation services including education on CKD progression, dialysis and home therapy options, access placement and self management strategies.
• Ensure teams execute consistent education, follow up, outreach, and engagement strategies across assigned physician practices and community locations.
• Maintain alignment across field-based and virtual workflows, ensuring seamless transitions between education, referral, and navigation pathways.
• Ensure all patient interactions-virtual or in-person-adhere to approved content, standards, and compliance requirements.

Market Strategy & Growth Ownership
• Own regional performance outcomes for early CKD education adoption, referral volume, patient engagement, and readiness-to-start metrics.
• Analyze referral trends, market penetration, geographic performance, and practice-level opportunities to guide strategic deployment of resources.
• Collaborate with regional and enterprise leadership to develop and execute market-specific growth strategies.
• Identify high-impact territories, underperforming markets, and emerging risks, adjusting strategy accordingly.
• Ensure alignment of community outreach efforts with provider engagement and value-based care goals.

Physician & Stakeholder Engagement
• Serve as an escalation point for high-impact physician practices and complex market relationships
• Support advocates in managing key accounts and navigating challenging stakeholder dynamics
• Reinforce the value proposition of early CKD education with providers, practice leadership, and internal partners
• Partner with operations, marketing, and clinical leaders to align provider engagement strategies

Operational Excellence & Program Integration
• Ensure field and virtual workflows are consistently executed and integrated smoothly across teams.
• Identify process barriers impacting patient engagement or provider participation and implement regional solutions.
• Maintain adherence to program standards, approved education practices, and quality expectations.
• Drive consistency, scalability, and high-quality execution across large, complex territories.

Reporting, Analytics & Continuous Improvement
• Track, analyze, and report regional performance metrics including:

o Referral trends

o Education engagement and completion rates

o Market and community activity
• Use data to inform coaching, strategy adjustments, and resource allocation
• Communicate performance insights and recommendations to senior leadership

PHYSICAL DEMANDS AND WORKING CONDITIONS:
• Day-to-day work includes desk and personal computer work, and interaction with patients, facility staff and physicians.
• The position requires frequent prolonged periods of sitting, standing, and computer use.
• Travel to business units, corporate meetings, and on-site training will be required.
• Travel to physician groups, healthcare locations, and associate home base.
• Must be willing to work occasionally in the evening and weekends, as needed.

SUPERVISION:
• Provides direct and/or indirect supervision of Kidney Care Specialist.
• Reports to Kidney Care and Value-Based Care executive leadership and partners closely with clinical, operational, and market leaders.

EDUCATION AND REQUIRED CREDENTIALS:
• High School diploma or G.E.D. required; bachelors preferred or 3-5 years clinical background preferred
• 2+ years of people management/leadership experience, preferably in a field-based or regional model
• Relevant licensure or certification preferred, as applicable.

EXPERIENCE AND SKILLS:
• Minimum of 3-5 years of experience in kidney care, chronic disease management, healthcare outreach, or related field
• Demonstrated success leading teams accountable for growth, adoption, or performance outcomes
• Strong ability to coach, influence, and develop high-performing teams
• Experience managing large, complex geographic regions with multiple stakeholders
• Excellent communication, presentation, and relationship-building skills
• Ability to interpret data and translate insights into action
• Valid driver's license with ability to travel within assigned market
• CKD education, dialysis, or transplant program experience
• Prior experience leading community-based or market-facing teams
• Background in program expansion, transformation initiatives, or growth strategies
• Familiarity with value-based care models and the role of education in improving outcomes
