As a Manager, Care Strategy and Operations, you will play a critical role in supporting our Oak Street Health patients, providers and business by owning the strategy and execution for two of our highest-priority clinical initiatives. You'll lead a small team while working at the intersection of our central functions ("Treehouse") and field-based clinical operations - translating program design into consistent, measurable delivery across Oak Street centers nationwide.
What You'll Do- Manage the programs. Lead strategy, execution, and continuous improvement for our Diabetes and Hypertension programs. Evaluate how they interact with broader Oak Street initiatives to drive clinical and economic results.
- Build and scale. Manage the full lifecycle of clinical interventions - from pilot design and launch through proof of concept and national scale.
- Lead a team. Manage and develop 1-3 direct reports, building capacity while maintaining accountability for program outcomes.
- Drive cross-functional alignment. Partner with clinical, operational, and regional leaders to build business cases and move initiatives forward without formal authority over those teams.
- Ensure field execution. Collaborate with regional teams to translate central strategy into consistent clinical and operational practice at the center level.
- Track what matters. Monitor and communicate key metrics - quality measures, utilization, and financial impact - and use data to drive program decisions.
What You Bring- 5+ years in healthcare strategy, consulting, population health, consulting, or clinical operations. Bachelor's degree required.
- Demonstrated ability to launch pilots and scale initiatives across a distributed, national footprint
- Strong project management skills across complex, multi-stakeholder environments
- Experience managing or mentoring others, with a collaborative leadership style
- Comfortable synthesizing data to build compelling business cases and influence senior stakeholders
- Thrives in ambiguous, high-growth environments; change-oriented without losing rigor
- Bachelor's degree required

Pay RangeThe typical pay range for this role is:
$66,330.00 - $145,860.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
