Full Job Description
We are looking for a leader who is committed to the health and well-being of the members we serve. As Manager, Care Management, you will lead a team of care management professionals who provide telephonic outreach, support, and care coordination services to our Special Needs members. We are looking for someone who can communicate with all levels of the organization and motivate team members to meet all established goals.

You will be responsible for management of the daily workflow and the leadership of the department staff. You will oversee the assessment and evaluation of members' needs and requirements to achieve an optimal wellness state. Use your management or supervisory skills, and your health care background to ensure that team members meet all productivity and quality standards. Someone who can lead change, promote a positive work environment and respond to unexpected changes or demands should be successful in this role.

Responsibilities include:

  • Provide managerial oversight of Humana associates in care management roles, including Care Managers and Care Coaches.
  • Provide leadership, encouragement and teaching of Humana Care Management techniques.
  • Monitor and audit staff performance.
  • Develop plans of action for process improvement
  • Help promote the multi-disciplinary healthcare team.
  • Ensure compliance and accuracy regarding clinical program design, implementation, monitoring and reporting.
  • Work with other members of the clinical leadership team and comply with policy and procedures, performance and reporting standards.

Required Qualifications

  • Associate's degree or higher

  • Active, unrestricted Registered Nurse (RN) license or Social Work (SW) license
  • 5 or more years of professional experience
  • 2 or more years of supervisory, operational leadership experience, including interim team lead, mentoring new associates, SME, project leader, etc.
  • Willingness to flex schedule availability, with evenings and occasional weekends

Preferred Qualifications

  • Bachelor's degree or advanced degree in nursing or business health field
  • Previous experience working in a managed care field
  • 5 or more years of previous management/supervisor level experience

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$86,300 - $118,700 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
