SUMMARY OF POSITION:

Responsible for managing, evaluating, providing and improving care coordination services for patients through efficient and effective planning, directing, coordinating and controlling of material/financial resources and human resources. This positions primary purpose is to manage the daily operations of case management and social work who are responsible for patient throughput, including, but not limited to: discharge planning; community resources utilization for patients; transfers-acute care, rehab, nursing home, home, and so forth; working with insurance companies; partnering with nursing staff, the patient/family and physician for effective and efficient delivery of care utilizing the patient?s available resources.

MINIMUM QUALIFICATION(S):

  • Master's of Social Work Degree or Bachelor's Degree in Nursing required.
  • 5 years experience in acute care required.
  • Progressive leadership experience required.
  • Either: (A) currently licensed to practice as a registered nurse by Pennsylvania Board of Nurse Examiners required, OR (B) currently licensed as a social worker by Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors required.

  • Organizational skills required.
  • Excellent written and oral communications required.
  • Excellent interpersonal skills and public speaking skills required.
