Full Job Description
The Manager, Care Coaching serves as the primary point of contact for Care Coaches and is responsible for maintaining team compliance. You will also collaborate with other departments. It is our priority to maintain members in the least restrictive environment. In addition to improving member's health and well-being. Reports to Regional Care Coach AD. The Manager, Care Coaching employs a variety of strategies, approaches and techniques to manage a member's health issues. In this role you will support a team of 15-17 Care Coaches, be accountable to overall team compliance, coaching, and accountability models. You will support metric driven reports and identify barriers and supports to overcome them. You will maintain caseload ratios, while minimizing mileage and overtime. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration and conducts briefings and area meetings; maintains frequent contact with other managers across the department. Must reside within 3 hours of Broward County. Use your skills to make an impact Required Qualifications • Bachelor's Degree in a health-related field • 2 or more years of managed care experience in Medicare and/or Medicaid. • 1 or more years of management/leadership experience. • 1 or more years of complex membership experience. • Proficiency in analyzing and interpreting trends • Progressive experience in the health solutions industry, with emphasis with leading and managing teams • This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits. Preferred Qualifications • RN or LPN with current State Licensure • Master's degree in human services, social sciences, or health related field. • Applicable State licensure in field of study • Proficiency in analyzing and interpreting financial trends • Previous experience in utilization management, discharge planning and/or home health or rehabilitation • Experience with health promotion, coaching and wellness • Health Plan experience • Previous Medicare/Medicaid Experience • Experience in maintaining NCQA standards. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $78,400 - $107,800 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
