DescriptionWeCare Centers at Foltsbrook Care Center is seeking an
experienced LPNAC to join our team on a
full-time basis. In this role, you'll support our nursing facility by completing MDS assessments and ensuring accurate documentation for quality resident care and proper reimbursement.
Key Responsibilities- Complete MDS assessments (MDS experience required)
- Prepare and transmit reports to Medicare & Medicaid
- Ensure accuracy of medical records and coding
- Conduct care area assessments and care planning
Skills, Knowledge and Expertise- Licensed Practical Nurse (LPN) in the State of NY with MDS experience
- Strong attention to detail and accuracy
- Ability to work independently and remotely
