Full Job Description
PURPOSE OF THE POSITION

Reporting to the Director of Care, and under general direction of Clinical Nursing Leaders, Nurse-in-Charge or RN, the Licensed Practical Nurse is responsible for the daily nursing care of residents, including medications, catheterizations and dressings, as well as routine feeding, personal hygiene and transporting of residents, and routine direction to Nursing Aides. Assesses, plans, implements and evaluates nursing care in accordance with established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia.

RESPONSIBILITIES

  1. Provides safe and respectful nursing care to an assigned group of residents according to established policies and procedures, in combination with evolving individualized care plans and setting care priorities.
  2. Performs nursing procedures such as catheterizations, dressings (sterile and non-sterile), blood pressures, temperature, pulse, respiration.
  3. Receives and processes doctors' orders according to policies and procedures. Transcribes orders onto appropriate patient/resident record and processes written orders as needed.
  4. Documents care provided such as assessments, status, progress, treatments, changes in condition, medications administered in manual and/or computerized environment.
  5. Dispenses and administers medications including narcotics (using eMAR system) as required, including oral, topical, subcutaneous injections and intramuscular injections according to established procedures and monitors medication's effects. Maintains communication with RN/RPN or other members of the interdisciplinary team as required.
  6. Assesses and monitors residents' actual and potential strengths and limitations including physiological, psychological, sociocultural and spiritual needs. Records observations as per facility guidelines/format (e.g. MDS, Point Click Care, Excelleris), and reports any significant changes/problems in resident's condition such as mobility, temperature, skin integrity, appetite and/or behaviour, to RN. or Nurse-in-charge.
  7. Provides personal care to residents, such as assisting with toilet needs, bathing, dressing, grooming, oral hygiene and care of skin/nails, as well as transporting residents within the facility if necessary.
  8. Responds to a variety of incidents such as Codes, injuries or falls, hazardous spills, aggressive behaviour and fire alarms. Provides emergency-response care until RN arrives in nursing unit. Reports incidents and fills out appropriate forms in accordance with established procedures.
  9. Ensures safety of residents during all tasks; reports any adverse events or near-misses to supervisor immediately.
  10. Assists with the orientation of new staff and student preceptors by performing duties such as demonstrating work procedures, proper use/location of equipment, sharing clinical knowledge using effective communication and acting as a positive role model. Provides leadership and routine direction to Nursing Aides as may be required.
  11. Participates in Interdisciplinary meetings, and attends Staff Development and In-service Education programs when required. Uses evidence-based resources such as Nursing Care Standards and drug reference guides, in-services and workshops, to keep nursing knowledge and procedures up-to-date.
  12. Performs related clerical duties such as answering the telephone, taking and relaying messages, and maintaining standardized records.
  13. Performs other related duties as assigned.


QUALIFICATIONS

  • Graduation from a recognized program for Practical Nurses including recent training in Pharmacology, Dementia-care and care of the elderly. current registration with British Columbia College of Nurses and Midwives (BCCNM).
  • Long Term Care / Gerontology training and experience is required.
  • Intermediate (Standard) First Aid and Basic Life Support from a WorkSafeBC approved provider is required.
  • PCC (Point Click Care) experience is preferred.
