Overview

CVP seeks Licensed Practical Nurses (LPNs) to join our team in providing our Nation's Veterans with skilled nursing care at the VA Hudson Valley Healthcare System, with services performed onsite at two locations: 2094 Albany Post Road, Montrose, NY and 41 Castle Point Road, Wappingers, NY. LPNs will provide care to patients in the Intermediate/Medical Nursing Home Care Unit and to Psychiatric patients, demonstrating increasing autonomy in patient care management and competence in caring for adult psychiatric, medical, and geriatric patients.

Multiple shift options are available, including day, evening, extended day, and overnight shifts.

Responsibilities

  • Provide skilled nursing care to patients across a variety of units including ambulatory care, critical care, nursing home care, and the Emergency Room, operating both independently and under the guidance and supervision of the Nurse Manager or their designee.
  • Prepare, administer, and accurately document commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications in accordance with approved local facility policies and procedures.
  • Observe and document patient responses to administered medications and any noted changes in patient condition, reporting findings to the supervising RN or physician in a timely manner.
  • Collaborate with physicians, registered nurses, and other members of the interdisciplinary healthcare team to ensure coordinated, high-quality patient care across all assigned units.
  • Perform and document nursing assessments within required timeframes and in accordance with facility policy and procedure, including head-to-toe assessments and vital signs monitoring.
  • Identify and act on significant or predictable changes in the patient's health condition, escalating concerns to the appropriate clinical staff as needed.
  • Maintain and update patient care plans to reflect current health status, interventions, and goals in alignment with the interdisciplinary care team.
  • Accurately document all patient care activities, assessments, and clinical observations in the VA's Computerized Patient Record System (CPRS) electronic health record in accordance with VHA Handbook 1907.01 and facility documentation standards.
  • Maintain patient confidentiality and a safe care environment in accordance with HIPAA, the Privacy Act, and all applicable VA regulatory requirements, including those set forth by The Joint Commission (TJC).


Qualifications

  • Must be eligible to obtain a government-sponsored background investigation. US Citizenship is required.
  • Must be a graduate of a state-approved school of practical or vocational nursing.
  • Must hold a current, active, full, and unrestricted license to practice as a Licensed Practical Nurse (LPN) in any State, Territory, Commonwealth of the United States, or the District of Columbia.
  • Minimum of one (1) year of clinical nursing experience as an LPN; experience in medical/surgical, geriatric, or psychiatric nursing settings is preferred.
  • Current Basic Life Support (BLS) certification issued by the American Heart Association.
  • Familiarity with electronic health record systems.
  • Demonstrated ability to communicate effectively in spoken and written English, including fluency in reading and writing clinical documentation.

Shifts
  • Day: 7:30 AM to 4:00 PM
  • Evening: 3:30 PM to 12:00 AM
  • Extended Day: 7:30 AM to 8:00 PM
  • Night: 7:30 PM to 8:00 AM

*Shifts are available 24/7, including weekends and Federal Holidays. Occasional overtime may be required.

Pay Rate
  • Hourly Pay: $40/hr
  • Health & Wellness (H&W): $5.09/hr (H&W will be prorated based on enrollment in the benefits offered, including automatic enrollment in employer-paid plans.)
