Licensed Physical Therapist - Cherrelyn Healthcare Center | Littleton, COPart-Time & Full-Time | $45-$52/hr DOEThe RoleYou'll carry a structured caseload of skilled nursing residents, developing and executing individualized PT plans that directly impact mobility, functional independence, and discharge outcomes.
Your day-to-day includes:- Initial evaluations and goal-setting for new admissions
- Therapeutic exercise, gait training, balance work, and functional mobility
- Transfer training and assistive device assessment
- Coordinating with nursing, OT, SLP, and care teams on shared resident goals
- Family education on home safety, carry-over exercises, and discharge prep
- Documentation in compliance with Medicare and facility standards

Requirements- Active Colorado Physical Therapy license in good standing
- Degree from an accredited PT program (DPT or equivalent)
- SNF or post-acute experience preferred - not required for the right candidate
- Strong documentation habits and communication skills
